In de jaren 70 wordt een Schotse arts per toeval door Idi Amin, destijds de nieuwe dictator van Oeganda, aangesteld als diens persoonlijke arts. Amin had namelijk vroeger bij het Britse leger getraind en een voorkeur ontwikkeld voor Schotten. Hij droeg regelmatig kilts en wilde een leger oprichten om Schotland onafhankelijk te maken. Aanvankelijk is de arts vereerd om deze positie te bekleden, maar al snel komt hij in gewetensnood, vanwege de vele gruwelijkheden die de dictator aanricht.
